Sports Physicals offered on July 7 & 14, 2010 from 1-4pm and August 9, 2010 from 7:30am-12:30 pm in Kingston
Darla Mays, PA-C of Kingston Family HealthCare will conduct sports physicals at her off ice located at 5988 State Street at a cost of $10 each; $5 of this cost will be donated back to Kingston School Athletic Program. Parents must accompany their child or send a “permission to treat” slip with the student and each participant must bring a sports physical form from the school office. No appointment necessary.
