Dr. Starbird is a graduate of Wayne State University School of Medicine. He completed his specialty training in family medicine through Oakwood Hospital's Family Practice Residency Program in Dearborn, Michigan. He is board certified in Family Practice, a diplomat of the National Board of Medical Examiners, and a fellow of the American Academy of Family Physicians.

The specialty of Family practice provides comprehensive health care for the entire family from newborns to the elderly. Family Practice places special emphasis on health maintenance and preventative medicine.

Please notify the receptionist if you are calling to make an appointment involving workmen's compensation or an auto accident. Bring all necessary information, such as employer authorization or auto insurance policy number, address, and claim number.
Please notify us 24 hours in advance if you are unable to keep an appointment so that we may accommodate other patients. Please note: appointments not cancelled or rescheduled will be charged.
FEES
Payment is expected when services are rendered. This includes co-pays. For your convenience, you may pay by cash, check, VISA, MasterCard credit card.
Please note: there is a charge for returned checks.
INSURANCE
We participate with Blue Cross/Blue Shield of Michigan, Community Blue, Blue Trust PPO, Aetna, Medicare, PPOM, Medicaid, Great Lakes, HealthPlan of Michigan, HealthPlus, as well as most commercial insurances. For your convenience, we will submit completed forms to the insurance company for you. However, any deductibles, co-pays, or uncovered services are your responsibility.
If your name, address, telephone number, or insurance coverage changes, please let us know as soon as possible. Thank you.
WORK/DISABILITY INSURANCE FORMS
Any forms submitted will require a minimum of one week to complete. Before turning any forms in to our office, patient information must be completed.
Please note: there may be a charge for completing forms.
PRESCRIPTIONS
When inquiring for medication refills, please allow 48 hours for telephone and 1 week for written prescriptions.
