Dr. Rees received his medical degree from the West Virginia School of Osteopathic Medicine. Dr. Rees completed his residency in Family Medicine at Trinity Terrace Park Hospital, Bettendorf, Iowa. Dr. Rees completed his undergraduate studies at Northern Michigan University.
Dr. Rees will provide primary care medical services to patients of all ages. His services will include but are not limited to care of acute illnesses, chronic disease management, preventative health care, well child care, adolescent care, men's and women's health care.

Monday - 10:00am - 7:00pm
Tuesday - 8:00am - 5:00pm
Wednesday - 8:00am - 5:00pm
Thursday - 8:00am- 12:00noon
Friday - 7:00am - 4:00pm

FEES
To keep billing costs down, payment is expected at time of service. This includes co-pays and deductibles. We accept cash, checks, VISA, MasterCard, or Discover credit card.
Please note: a $25 non-sufficient fund charge is placed on all returned checks.
We will be happy to discuss our fees and payment policies with you.
INSURANCE
We participate with Blue Cross/Blue Shield of Michigan, Community Blue, Blue Trust PPO, Aetna, Medicare, PPOM, Medicaid, Great Lakes, HealthPlan of Michigan, HealthPlus, as well as most commercial insurances. For your convenience, we will submit completed forms to the insurance company for you. However, any deductibles, co-pays, or uncovered services are your responsibility.

PRESCRIPTIONS
For your safety, prescriptions can only be refilled if your examination is current. When inquiring for medication refills, Please allow 24 hours for telephone and written prescriptions. |
