      By Appointment Monday through Friday. To schedule an appointment, please call 810-346-2751.

      All appointments involving auto or worker's compensation must have the proper information presented at the time of your appointment. This includes employer's authorizations, auto insurance policy and claim numbers, and the name and telephone number of the contact person.

      Please notify us 24 hours in advance if you are unable to keep an appointment. There may be a charge for appointments that are not cancelled or rescheduled.

      FEES
      Payment is expected at time of visit. This includes co-pays and deductibles. For your convenience, you may pay by cash, check, VISA, MasterCard, American Express, or Discover credit card.

      Please note: there is a charge for all returned non-sufficient funds checks.

      INSURANCE
      We participate with Blue Cross/Blue Shield of Michigan, Community Blue, Blue Trust PPO, Aetna, Medicare, PPOM, Medicaid, Great Lakes, HealthPlan of Michigan, HealthPlus, as well as most commercial insurances. For your convenience, we will submit completed forms to the insurance company for you. However, any deductibles, co-pays, or uncovered services are your responsibility.

      If your name, address, telephone number, or insurance coverage changes, please let us know as soon as possible. Thank you.


      When inquiring for medication refills, please allow 24 hours for telephone and 72 hours for written prescriptions.
