Galen Smith is a graduate of the University of Nebraska Medical Center. He is a retired Air Force Captain and completed training in the United States Air Force Physician Assistant Training Program. He is board certified as a physician assistant and is a fellow of the American Academy of Physician Assistants and the Michigan Academy of Physician Assistants.
Dr. Starbird is a graduate of Wayne State University School of Medicine. He completed his specialty training in family medicine through Oakwood Hospital's Family Practice Residency Program in Dearborn, Mich. He is board-certified in family medicine, a diplomat of the National Board of Medical Examiners and a fellow of the American Academy of Family Physicians.
To keep billing costs down, payment is expected at time of service. This includes co-pays and deductibles. We accept cash; checks; and VISA, MasterCard or Discover credit card.
Please note: A $25 nonsufficient fund charge is placed on all returned checks.
We will be happy to discuss our fees and payment policies with you.
We participate with Blue Cross Blue Shield of Michigan, Community Blue, Blue Trust PPO, Aetna, Medicare, PPOM, Medicaid, Great Lakes, HealthPlan of Michigan and HealthPlus, as well as most commercial insurances. For your convenience, we will submit completed forms to the insurance company for you. However, any deductibles, co-pays or uncovered services are your responsibility.
If your name, address, telephone number or insurance coverage changes, please let us know as soon as possible.
For your safety, prescriptions can only be refilled if your examination is current. When requesting medication refills, please allow 24 hours for telephone and written prescriptions.
