Please notify the receptionist if you are calling to make an appointment involving workers' compensation or an auto accident. Bring all necessary information, such as employer authorization or auto insurance policy number, address and claim number.
Please notify us 24 hours in advance if you are unable to keep an appointment so that we may accommodate other patients.
To keep billing costs down, payment is expected at time of service. This includes co-pays and deductibles. We accept cash; checks; and VISA, Mastercard and Discover credit card.
Please note: A $25 nonsufficient fund charge is placed on all returned checks.
We will be happy to discuss our fees and payment policies with you.
We participate with Blue Cross Blue Shield of Michigan, Community Blue, Blue Trust, PPO, Aetna, Medicare, PPOM, Medicaid, Great Lakes, HealthPlan of Michigan and HealthPlus, as well as most commercial insurances. For your convenience, we will submit completed forms to the insurance company for you. However, any deductibles, co-pays or uncovered services are your responsibility.
If your name, address, telephone number or insurance coverage changes, please let us know as soon as possible.
For your safety, prescriptions can be refilled only if your examination is current. When requesting medication refills, please allow 24 hours for telephone and written prescriptions.
