Our office specializes in family medicine. We provide the following services for the entire family:

  • Allergy injections.
  • Blood draws for testing.
  • Breast and cervical cancer prevention program.
  • Immunizations.
  • Minor diagnostic testing.
  • Minor surgical procedures.
  • Newborn to geriatric health care.
  • Sports, school, employment, insurance and DOT physical exams.
  • Preventive medicine.
  • Treatment for acute health problems.
  • Treatment for chronic health problems.

Office hours

By appointment Monday through Friday. To schedule an appointment, please call 810.346.2751.

  • Monday, Tuesday, Wednesday, Friday: 8 a.m. to 5 p.m.
  • Thursday: 8 a.m. to 7 p.m.
  • Closed 1 to 2 p.m. for lunch.

All appointments involving auto or workers' compensation must have the proper information presented at the time of your appointment. This includes employer's authorizations, auto insurance policy and claim numbers, and the name and telephone number of the contact person.

Please notify us 24 hours in advance if you are unable to keep an appointment. There may be a charge for appointments that are not canceled or rescheduled.

Fees

Payment is expected at time of visit. This includes co-pays and deductibles. For your convenience, you may pay by cash; check; and VISA, MasterCard, American Express or Discover credit card.

Please note: There is a charge for all returned checks.

Insurance

We participate with Blue Cross Blue Shield of Michigan, Community Blue, Blue Trust PPO, Aetna, Medicare, PPOM, Medicaid, Great Lakes, HealthPlan of Michigan and HealthPlus, as well as most commercial insurances. For your convenience, we will submit completed forms to the insurance company for you. However, any deductibles, co-pays or uncovered services are your responsibility.

If your name, address, telephone number or insurance coverage changes, please let us know as soon as possible.

Prescriptions

When requesting medication refills, please allow 24 hours for telephone and 72 hours for written prescriptions.
